Community Health Improvement
Planning (2006-2011)

 

Illinois Project for Local Assessment of Needs (IPLAN)

is a community needs assessment and planning process that each local health department in the State of Illinois is mandated to conduct every five years. The Knox County Community Health Improvement Plan is the result of the participation and input of many community individuals, agencies, and organizations. It is intended to assist community participants in better addressing those

priority health needs through the application of

developed strategic planning initiatives and appropriate programming.

 

In 2010, the Knox County Health Department in cooperation with the Healthy Communities Project Partners initiated community health improvement planning using community health strategic planning process called:

Mobilizing for Action through Planning and
Partnerships  (MAPP).

This process is intended to identify common priorities, goals and strategies, and to facilitate collaborative efforts to improve the health and well-being of the community.

As part of the MAPP process, the Health Department, in partnership with Project partners and community leaders

is conducting the following assessments in an effort to develop a revised community health improvement plan

for Knox County.

Community Health Status Assessment (CHSA)
Community Themes and Strengths Assessment (CTSA)
Forces of Change Assessment (FOCA)
 

It is expected that the new community health improvement plan will be completed in late 2011 for the upcoming five-year period (2011-2016).
